1. Introduction

Welcome to the Be.Well by Slade360° platform ("Platform", "Service"). These Terms and Conditions ("Terms") govern your use of the Platform provided by Savannah Informatics Limited ("Savannah", "we", "us", "our").


By accessing or using the Platform, you agree to be bound by these Terms. If you do not agree to these Terms, please do not use the Platform.

2. Definitions

  • "User" refers to any individual who accesses or uses the Platform
  • "Services" refers to all services provided through the Platform including health information, facility search, appointment booking, and health navigation
  • "Health Provider" refers to any healthcare facility, doctor, or medical professional listed on the Platform
  • "Content" refers to all information, data, text, images, and materials available on the Platform

3. Eligibility

To use the Platform, you must:

  • Be at least 18 years old or have parental/guardian consent
  • Have the legal capacity to enter into binding contracts
  • Provide accurate and complete registration information
  • Maintain the security of your account credentials

4. User Account

When you create an account on the Platform:

  1. You are responsible for maintaining the confidentiality of your account information and password
  2. You agree to accept responsibility for all activities that occur under your account
  3. You must notify us immediately of any unauthorized use of your account

6. Health Information Services

The Platform provides health information and navigation services. However:

  1. Not Medical Advice: The information provided on the Platform is for informational purposes only and does not constitute medical advice, diagnosis, or treatment.
  2. No Doctor-Patient Relationship: Use of the Platform does not create a doctor-patient relationship between you and Savannah or any Health Provider listed on the Platform.
  3. Consult Healthcare Professionals: Always seek the advice of qualified healthcare professionals with any questions you may have regarding a medical condition.
  4. Emergency Services: The Platform is not designed for emergency medical situations. In case of a medical emergency, call emergency services immediately.

7. Appointment Booking

When using our appointment booking features:

  • Appointment confirmations are subject to Health Provider availability
  • You must provide accurate information when booking appointments
  • You are responsible for attending scheduled appointments or canceling in advance
  • Health Providers may have their own terms and conditions that apply to appointments

8. Insurance Verification

The Platform may help verify your health insurance coverage. However:

  • Insurance information provided is based on data from insurance providers and may not always be current
  • You are responsible for verifying your coverage directly with your insurance provider
  • We are not responsible for any discrepancies between displayed information and actual coverage

9. Intellectual Property

All content on the Platform, including text, graphics, logos, images, and software, is the property of Savannah Informatics Limited or its licensors and is protected by intellectual property laws.


You may not:

  • Copy, modify, distribute, or create derivative works from any content on the Platform without our express written permission
  • Use any content for commercial purposes without authorization
  • Remove or alter any copyright or proprietary notices

  • Issues arising from interactions with Health Providers listed on the Platform

13. Indemnification

You agree to indemnify and hold harmless Savannah Informatics Limited, its affiliates, and their respective officers, directors, employees, and agents from any claims, liabilities, damages, losses, and expenses arising from:

  • Your use of the Platform
  • Your violation of these Terms
  • Your violation of any rights of another party
  • Any content you submit or transmit through the Platform

14. Third-Party Links

The Platform may contain links to third-party websites or services that are not owned or controlled by Savannah. We are not responsible for the content, privacy policies, or practices of any third-party websites.

15. Modifications to Terms

We reserve the right to modify these Terms at any time. We will notify you of any material changes by posting the new Terms on the Platform and updating the "Last Updated" date. Your continued use of the Platform after such changes constitutes your acceptance of the new Terms.

16. Termination

We may terminate or suspend your access to the Platform immediately, without prior notice, for any reason, including if you breach these Terms. Upon termination:

  • Your right to use the Platform will cease immediately
  • We may delete your account and all associated data
  • All provisions that should survive termination shall continue to apply
